Neurology on the Hill Online Application

This form must be completed and submitted online in order to apply to participate in Academy's Tenth Annual Neurology on the Hill (NOH) in Washington, DC on February 26 to 28, 2012.

Eligibility

This event is open to active AAN Members in good standing with a home address in the United States. Applicants must have a strong interest in advocating on behalf of their profession and the interests of their patients. Students and Coresponding members are not eligable for this program.

Participant Expectations

By completing this application you attest to your ability to be in Washington, DC on February 26 to 28, 2012. Should your application be accepted, your air fare and lodging costs will be covered by Academy. Participants will be responsible for a modest registration fee to help offset the costs of the program. This fee will be $50 for residents, fellows, and members located in the local Washington, DC area and $150 for all others. Since the Academy does cover your travel expenses, all travel arrangements must be made through the Academy. Please note that the Academy's primary form of communication with NOH applicants occurs via email.

Instructions

• Your contact information has been automatically filled in with information from the Academy member database. Please review it to make sure it is up to date and enter any changes. All information, including your notification letter, will go to this address.
• The demographic information is optional, but will help us better understand the characteristics of members applying for this program.
• The application asks for two narrative questions:
• Why do you want to attend Neurology on the Hill?
• Please describe any past experience you have meeting with lawmakers (state or federal) or staff members?

Tips

• Save your narrative answers in a Word document in case there is any problem with the electronic submission process.

• You may click the save button at any time to save your progress. You can choose to save your application and return to it as many times as needed up until the deadline. However, once you click submit, you will no longer have access to your application.

Applications must be received by the Academy by 11:59pm CST, Sunday, December 11, 2011 in order to be considered for the 2012 Neurology on the Hill.
