Science CME Request Form

The Science CME Request form is online!

In order to properly record your Scientific CME credits and give feedback to enhance the meeting, please follow these steps:

1. Access the Science CME Request form

2. Log in using your Registration ID and AAN ID. Both of these IDs can be found on your badge, your tickets, or in your registration confirmation email.
3. Click the link for the "Demographics and Scientific Program Evaluation Survey."
4. Complete the evaluation questions in the survey and click the "Finish" button.
5. At the thank you screen, click the link to "Return to Survey Menu Screen" to complete your session evaluations.
6. Click the link for the first program you wish to evaluate.
7. While in the individual program, select the number of minutes you spent at the program and complete the evaluation. Click the "Submit" button once you have completed the information for the program.
8. Click the "Continue" button to return to the main program selection screen.
9. Repeat steps 6 through 8 until you have completed the evaluation for each program attended. At the main program selection screen, you may end the session by scrolling to the bottom and clicking the "End My Session" button.
10. Click the link to close the window and end your session.

If you wish to review and edit the evaluations that you have already completed, you may do so by clicking the "Lookup Completed Evaluations" link.

How long will the site be open?

The Scientific CME request form will be available until May 21, 2011.

The education programs that I registered to attend aren't listed on the site. How do I get CME for those?

CME for programs that required you to pre-register to attend is tied to the program registration and ticket that was turned in at the door of the program. There is no need to manually record the number of minutes attended for these programs.

I'm trying to print out my CME transcript from the website and it says 0 hours. Did I record my information correctly?

For AAN members, the online CME transcript on aan.com will be updated and available 6-8 weeks after the meeting. For non-members, a CME certificate will be mailed to you 6-8 weeks after the meeting.
