AAN Payer Predicament Form

The American Academy of Neurology has developed the Payer Predicament Form (PPF) as a tool for helping members address issues with private insurance companies. The Payer Predicament Form is submitted online directly to Academy staff who will track issues and identify areas where greater payer advocacy is needed.

Helpful Hints for Using the Payer Predicament Form

  • The AAN suggests the person designated to collect issues for your practice be the person who will handles and submits claims to the private insurance companies on a regular basis.
  • Complete a Payer Predicament Form for each issue that arises with a specific private insurance company. The form is very user-friendly; and each issue can be easily identified. Additional space is available to explain the issue in as much detail as is necessary. If you would like to be contacted about the issue by Academy staff, please be sure to fill out the designated area at the bottom of the form with your name, e-mail address and phone number.
  • If you are experiencing an issue from more than one private insurer, please submit one form per insurer. If you are experiencing more than one issue from a single insurer, please submit all issues on one form.
  • The Academy would appreciate receiving any data that your office collects, should you decide that you would like to create your own version of the AAN Payer Predicament Form. Please forward any information to ckokaisel@aan.com.
