In order to participate in Medicare, it is vital that physicians be able to navigate the Medicare enrollment process. Due to constant changes in the program, physicians face many challenges in keeping up with requirements for enrolling and participating. The Academy is therefore encouraging members to utilize an American Medical Association (AMA) form for physicians to complete regarding Medicare enrollment issues with CMS. Download this form

The AMA has dedicated an entire webpage to address various challenges and complex procedures involving Medicare enrollment. Physicians are encouraged to use this page to obtain information on the enrollment application and process and also information to help ensure the current enrollment record is up to date. Many resources including CMS Final Rules and Comment Letters, Medicare Enrollment Tips, and more are available on this webpage.

To report an issue, send the form to MedicareProblems@ama-assn.org. For more information, visit the AMA's Medicare Enrollment webpage.
