Improved Functionality, Satisfaction, and Global Impression of Change With Ubrogepant for the Acute Treatment of Migraine in Triptan Insufficient Responders

Triptan insufficient responders are identified by suboptimal efficacy, side effects, or contraindications that preclude the use of triptans.
To examine the impact of ubrogepant on functional disability, medication satisfaction, and global impression of change in triptan insufficient responders.
ACHIEVE I and II were multicenter, double-blind, single-attack phase 3 trials. Adults with a history of migraine were randomized 1:1:1 to placebo or ubrogepant (50 mg or 100 mg, ACHIEVE I; 25 mg or 50 mg, ACHIEVE II). At baseline, participants were categorized as triptan responders, triptan insufficient responders, or triptan naive. Outcome measures included a functional disability scale (FDS), patient satisfaction with medication, and Patient Global Impression of Change (PGIC). This analysis used multivariable logistic regression and pooled data for placebo and ubrogepant 50 mg in the triptan insufficient responder subgroup.

There were 1799 participants (n=912 placebo, n=887 ubrogepant 50 mg) included in the pooled analysis. At baseline, 451 (25%) were categorized as triptan insufficient responders. At 2 hours post-dose, a significantly higher percentage of triptan insufficient responders treated with ubrogepant vs placebo were able to function normally on the FDS (38.2% vs 28.7%; OR: 1.57, 95% CI: 1.01, 2.45; P=0.046); were satisfied/extremely satisfied with trial medication (33.0% vs 20.9%; OR: 1.84, 95% CI: 1.19, 2.83; P=0.006); and indicated their migraine was much better/very much better based on the PGIC (30.5% vs 18.2%; OR: 2.00, 95% CI: 1.22, 3.29; P=0.006).

In people with migraine who are triptan insufficient responders, in comparison with the placebo arm, individuals treated with ubrogepant had favorable 2-hour outcomes, as measured by the FDS, patient satisfaction with medication, and PGIC.
