Use of Medical Education Program to Identify Practice Gaps Among Neurologists in the Use of Preventive Therapies for the Management of Migraine

Migraine headache affects at least 39 million men, women, and children in the United States with more than 4 million experiencing at least 15 migraine days per months. The more days per month a person experiences migraine, the higher the burden of disease. Despite a wide variety of preventive migraine therapies, such therapies are substantially underutilized in practice. 
The goal of this study was to obtain a better understanding of the specific nature of gaps in knowledge and competence for the use of preventive migraine therapies among neurologists. 

A CME-certified clinical practice assessment consisting of 27 multiple-choice questions on knowledge, clinical preferences, and competence was made available to neurologists in the United States. The questions were based on clinical trials, guidelines, and expert faculty recommendations regarding the appropriate use of preventive migraine therapies. The self-assessment posted on the Medscape Education website on March 26, 2020, with responses collected through April 21, 2020.

A total of 151 neurologists completed the self-assessment during the study period. 20% of neurologists were unable to correctly answer questions pertaining to outcomes from the CaMEO study of migraine burden. When asked about the use of the 2019 AHS Position Statement on new migraine treatments to guide clinical decision-making, only 54% of neurologists answered correctly. Neurologists were particularly unaware of clinical trial outcomes for anti-CGRP monoclonal antibodies, with only 41% selecting a correct answer.  Lastly, only 37% of neurologists correctly answered questions relevant to the clinical selection and use of anti-CGRP monoclonal antibodies.

This research yielded important insights into current clinical knowledge and competence gaps of neurologists regarding the use of preventive therapies for the management of migraines The results indicated a need for continued education on the clinical profile of patients who are candidates for and the personalized selection of preventive migraine therapies.
