Joint Hypermobility Syndrome in Children and Adolescents with Headache: a retrospective study

 Studies conducted in adults with headaches suggested that headache disorders are 3.2 times more prevalent in patients with JHS when compared to patients without JHS. However, no similar data are available for children and adolescents with headaches. 

To determine the association between pediatric headache and joint hypermobility syndrome (JHS).

Retrospective database query and chart review of patients seen in the Headache Center at Cincinnati Children’s Hospital from 2015-2020. As part of the initial visits, patients presenting to the Headache center were assessed for hypermobility using the Beighton score with JHS defined as a Beighton score≥ 6. Headaches were diagnosed based on International Classification of Headache Disorder.

A total of 1657 patients with JHS and 3727 controls were included in this study. Results indicated that 92% patients were diagnosed with migraine without aura, 22% patients had migraine with aura and 18% patients had medication overuse headache. The prevalence of chronic migraine was noted to be higher in patients with JHS when compared to patients without JHS (55.8% vs 51.6 %). Females were more commonly diagnosed with JHS (77%) as compared to males (23%). Average frequency of headaches in patients with JHS was significantly higher (16.7±9.9) compared to those without JHS (15.9±10.2) (p<0.005). Nausea (73% vs 67%), photophobia (87% vs 84%), phonophobia (82% vs 77%), PedMIDAS headache related disability (48% vs 41%) and anxiety (19% vs 16%) were noted to be statistically more prevalent in patients with JHS (all p<0.001). Clinically, patients with JHS were noted to have frequent temporomandibular joint popping (17.5%) as opposed to patients without JHS (7%).

JHS is strongly associated with migraine in children and adolescents. JHS was noted to be more prevalent in females as compared to males. Headache disability burden was higher in patients with JHS when compared with patients without JHS.
