Title Long-Term Safety and Tolerability of STS101, A Novel Investigational Dihydroergotamine Nasal Powder: Initial Data from the ASCEND Study

Background STS101, a novel investigational DHE nasal powder delivered via a patient-friendly device, is being investigated in a Phase 3 efficacy study (SUMMIT; STS101-007; NCT 04940390) and a long-term, open-label safety study (ASCEND; Study STS101-003; NCT 04406649).

Objective Open-label, long-term Phase 3 study evaluating the safety and tolerability of STS101 in the acute treatment of migraine attacks over 12 months.

Design/Methods ASCEND is an ongoing, open-label, 12-month study in subjects with migraine. Study subjects may use up to 2 doses of STS101 5.2 mg within 24 hours and up to 12 doses per month. Safety evaluations include physical examinations, vital signs, nasal examinations, electrocardiograms, laboratory tests, and adverse event (AE) assessments. Preliminary data is presented here.

Results The safety population included 273 subjects who treated 4,247 migraine attacks. The population mean age was 39 ± 11 years, 89% female, and 84% Caucasian. 20 subjects (7.3%) discontinued the study due to AEs including 8 (2.9%) associated with nasal AEs. Two subjects had serious AEs unrelated to STS101 (postural orthostatic tachycardia syndrome; cholecystitis). The most frequent AEs reported by subjects were nasal discomfort (13.9%), dysgeusia (7.7%), and nasal congestion (6.2%). Most AEs were mild and transient, and no serious AEs were related to STS101. An optimized STS101 device was introduced during the study, and no differences in AE rates or severity were observed between the original and optimized devices. Nasal examinations, ECGs, vital signs showed no clinically significant differences in the follow up evaluations.

Conclusions STS101 was well tolerated by subjects with migraine when used long-term and on a PRN basis.
