Title Reliability of Reachable Workspace (RWS) Measurement System in a Large, Multi-Site, International FSHD Clinical Study

Background The reachable workspace (RWS) assessment system utilizes motion capture technology (Kinect sensor) to quantitatively track upper extremity skeletal motion and reconstruct an individual's upper-extremity reachability. The developed RWS outcome measure has shown excellent reliability in FSHD. However, much of the development and testing has been done at one site with small sample sizes.

Objective

Determine reliability of the RWS system in a large, multi-site, international FSHD clinical study (1 E.U. and 8 U.S.-based ReSolve sites).

Design/Methods Individuals with FSHD at each site underwent RWS assessment two times over two consecutive days for each arm (left and right), and during normal (unweighted) and weighted (500g wrist weight) conditions. Average difference between the two days, measurement error (SEM) and intra-class correlation (ICC) analyses were performed. RWS assessment is expressed as relative surface area (RSA) using 4 frontal quadrants (Q1, Q2, Q3, Q4) and summated totals (Q1-Q4).

Results

A total of 167 unique patients across all sites were available for analyses. Majority (72%,) of participants had total RSA values between 0.30 and 0.70 with an average total RSA of 0.49 (SD= 0.14) and 0.50 (SD=0.13) for right and left arms, respectively, upon enrollment. Mean total RSA (SEM) differences between appointments for the right and left arm were 0.015 (0.05) and 0.006 (0.04), respectively, for unweighted and 0.005 (0.04) and 0.008 (0.04) for weighted assessments. ICCs for Q1, Q3, Q4 and total values were consistently higher than 0.90 (all p < 0.001) for all conditions, with Q2 being the only quadrant to dip to ~0.85 (p < 0.001).

Conclusions Reliability of the RWS system for large, multi-site studies remains consistently high, with low measurement error, across conditions and arms in individuals with FSHD. These results increase confidence in its use for clinical practice and/or trials in FSHD.
