Vestibular Paroxysmia Successfully Treated with N-acetylcysteine: A Case Report

Vestibular paroxysmia is a rare disorder characterized by stereotyped episodes of vertigo that are frequent and short-lasting often with associated tinnitus, aural fullness, incoordination, imbalance, or hearing loss. The etiology is unknown, but presumably is a result of ephaptic discharges from the proximal 8th cranial nerve following nerve injury. Treatment with carbamazepine or oxcarbazepine is diagnostic, as well as, therapeutic, but is not always a feasible treatment option.

To describe a case of probable vestibular paroxysmia that was successfully treated with N-acetylcysteine. 
A case report. 
We describe a 46-year-old woman with anxiety, hypothyroidism, menstrual-related migraine well-controlled on oral combined contraceptive, migraine with visual aura, and migraine aura without headache who presented for new-onset episodes of paroxysmal vertigo lasting 1-2 minutes and occurring up to 50 times per day with preceding left ear tinnitus. Episodes occurred and resolved spontaneously without identifiable provoking factor or position. There was no prior nidus event, aural fullness, facial weakness, vision changes, incoordination, or imbalance. Audiogram demonstrated mild bilateral hearing loss at 3000 Hz, and slightly asymmetric low frequency hearing loss in the left ear although within normal range. Vestibular testing demonstrated nystagmus with positional testing along with abnormal auditory brainstem response in the left ear and normal vestibular evoked myogenic potentials. MRI brain with and without contrast including internal auditory canal protocol showed no abnormal contrast enhancement, mass, or lesion, but demonstrated multiple white matter FLAIR hyperintensities along with a vein abutting the posterior aspect of the left 8th cranial nerve. She was treated with N-acetylcysteine 500 mg twice per day with resolution of her symptoms. 
N-acetylcysteine should be considered an alternative treatment for vestibular paroxysmia especially when comorbidities or patient preference preclude treatment with oxcarbazepine and carbamazepine.
