Improve safety with AAN information on abuse and violence, drug and device safety, and patient safety education. Additional tools can be found at the Agency for Healthcare Research and Quality (AHRQ) for improving safety at the point of care and during handoffs and sign outs.
Improve Patient-Centered Care
Many organizations have resources to promote patient-centered care. Most notably, the Institute for Healthcare Improvement has a Patient and Family Centered Care Organizational Self-Assessment Tool. This tool allows organizations to assess their performance of specific components of patient centered care.
The Agency for Healthcare Research and Quality has a Health Literacy Universal Precautions Toolkit. This tool is designed to help practices meet the needs of their patients and increase knowledge of health information. Another patient-centered tool is the Consumer Assessment of Healthcare Providers and Systems (CAHPS®) survey. It can be used by providers and health systems to ask patients about their healthcare experience after a visit.
The Department of Defense (DoD) and the Agenda for Healthcare Research and Quality (AHRQ) co-developed a tool, TeamSTEPPS, to improve collaboration and communication about patient safety among healthcare providers.
The American College of Physicians (ACP) published a position paper in 2013 on clinical care teams. This paper provides a framework for team health care delivery.
