Pruritus as initial symptom of Neuromyelitis Optica Spectrum Disorder. A Peruvian case report

To  report  the pruritus as an initial symptom of Neuromyelitis Optica Spectrum Disorder(NMOSD) in a Peruvian affected individual.

NMOSD is an inflammatory demyelinating disease that manifests with  three essential features: transverse myelitis, optic neuritis, and area postrema syndrome. Less frequent associated symptoms include diencephalic, brainstem and cerebral regions as well as symptomatic narcolepsy. Pruritus and paresthesia have been described as initial symptom in 27.1% longitudinally extensive transverse myelitis (LETM) cases with 87.5% of them also being positive for AQP4 antibodies
Detailed clinical assessment. Spinal cord MRI including contrast-enhanced T1 and T2 protocols. IRB approval obtained from the Instituto Nacional de Ciencias Neurologicas.  

A 23 years-old female without significant past medical history presented with a 2-week history of pruritus followed by paresthesia  and left arm hypoesthesia. The physical exam was significant for excoriated skin lesions in the right submaxillary region, unilateral left-sided ptosis, left hemiparesis , hyperreflexia in the all the extremities, a positive bilateral Hoffman sign, a positive left Babinski sign, left upper extremity and right lower extremity hypoesthesia. Spinal magnetic resonance imaging (MRI) showed a central lesion from C1 to C6, extending to the area postrema with minimal contrast enhancement .

During the course of her hospitalization patient also developed nausea, vomiting and urinary retention. Given her clinical presentation, the MRI lesions with a LETM  associated and her aquaporin-4 antibody-positive, the patient was diagnosed of NMOSD.

This is the first NMOSD case report   in peruvian patient,  experiencing  pruritus  as the initial symptom. Pruritus is related to spinal cord injuries and could indicate the onset of acute myelitis and may be a under-recognized characteristic of myelitis associated with NMO.
