Pediatric Shigellosis Encephalopathy with Atypical Brain Imaging

Shigellosis is a gastroenteritis disease caused by a class of bacteria known as Shigella. In addition to the typical gastrointestinal symptoms, shigellosis can be associated with neurological extraintestinal manifestations such as encephalopathy and seizures. In this case report, we present a case of a previously healthy five-year old girl who was brought to the hospital for seizures and persistent encephalopathy, after having two days of fever, nausea, vomiting, and non-bloody diarrhea from shigellosis. Our magnetic resonance imaging of the brain revealed a diffuse cortex sparing pattern of abnormal diffusion restriction not commonly associated with shigellosis. 

We present a comprehensive case report of a previously healthy 5-years-old girl who suffered from shigellosis encephalopathy and was found to have diffuse cortex sparing pattern of abnormal diffusion restriction not commonly associated with shigellosis.

We provide a comprehensive report of her clinical course during hospitalization including her neurologic examination findings, brain imaging, electroencephalogram, relevant laboratory results, management, and her post-hospitalization course.
